You're definitely right to say to "also have the latest security patches, virus definitions, spyware updates" - just last week I saw an article about some sites that tried to steal passwords stored in IE by some security hole. Since web browsers are generally the route of entry for a lot of bad software, it pays to give close attention to your browser security settings too.
Incidentally, HiJackThis is a wonderful tool for notifying you about all new things starting up on your PC automatically (even services) and new things integrated in IE. It doesn't use a database so you need to know what you're doing to a certain extent, but it does mean it's powerful (I've been using it over a year). I put a shortcut in my startup folder and use a scheduler to run a check at the end of the day too (it doesn't run in the background):
