You can use the settings (the gear icon in the toolbar at the top of the CHM) to select a tab you want starting with and other things.

I forgot about that. I had a hotkey to preselect the index tab, but it seems one of the recent changes broke it. When the help file first loads, the Alt+N and Alt+S hotkeys do not work unless I click the page first. However, if I set the index tab as default, Alt+C and Alt+S work just fine. Perhaps it is because the input box takes focus.
